Our Registration Packets are available through this website in a pda. format.

If you click on the link below it will then give you the opportunity to print the packet.

Please fill out all appropriate information and then deliver it to our office for a interview and viewing of our campus.

If you need the packet faxed to you, fill out the contact CCS page with a packet information fax request and our staff will get the packet faxed as soon as possible.

Positions fill fast so get your packet returned as soon as possible.
